There's no separate "renewal-only" form — you complete the same intake, select "Renewal" as your status, and schedule a telemedicine visit. If certification is still clinically appropriate, a new certification is issued.
Certification terms
New York certifications may run for up to two years (per New York State Senate Bill S3294A, signed November 2025, effective approximately February 2026). Choosing the 2-year option at renewal means one less visit to schedule down the road.
No card, no state fee
There is no state registration fee and no physical card to carry. Your renewed certification updates your existing registry ID.
